Haw. Code R. § 16-25-8 - Application forms; fees
(a) Each
application referred to in this chapter shall be made by the completion and
submittal to the commissioner, in duplicate, of:
(1) The application form prescribed by the
commissioner, copies of which may be obtained by request from the division;
or
(2) The application in letter
form, if no application form has been prescribed by the commissioner. The
application in letter form shall state in detail the nature and scope of the
requested consent or approval and all information reasonably required for the
commissioner's review and consideration of the application.
(b) If in respect of the subject
matter of an application the applicant is required to submit an application to
the appropriate federal regulatory agency on a prescribed form, the application
filed with the commissioner may constitute a copy of the application form filed
or to be filed with the respective federal agency. The commissioner may require
in respect of this type of application the submittal of information not
specifically prescribed by the federal application form, but required by state
law.
(c) An application fee shall
be paid by check made payable to the "Department of Commerce and Consumer
Affairs, State of Hawaii", concurrently with the filing of the application. The
fee shall be nonrefundable.
