Haw. Code R. § 16-53-15.5 - Cemetery and funeral trusts

The fees for cemetery and pre-need funeral authorities shall be as provided in this section:

(1) Application fee	$ 100
(2) Original license fee	$ 200
(3) License fees:
(A) First year of the biennium (original license fee, plus one-half of the biennial renewal fee)	$ 400
(B) Second year of the biennium (original license fee)	$ 200
(4) Biennial renewal fee	$ 400
(5) Restoration fee for failure to renew license shall be the penalty fee of $ 40, in addition to delinquent renewal fees.
(6) Reinstatement fee for a suspended license shall be the penalty fee of $ 200, in addition to delinquent renewal fees.
(7) Reissuance of license fee	$ 10
(8) Compliance resolution fund:
(A) Compliance resolution fund fee	$ 55
(B) Annual compliance resolution fund fee	$ 55
(C) Biennial compliance resolution fund fee	$ 110
