The fees for the board of dental examiners shall be as provided in this section:

(1) Application fee	$ 30
(2) Examination and reexamination fee	$ 200
(3) Examination handscore fee	$ 20
(4) Examination review fee	$ 25
(5) Original license fee	$ 40
(6) License fees:
(A) First year of the biennium (original license fee, plus one-half of the biennial renewal fee)	$ 60
(B) Second year of the biennium (original license fee)	$ 40
(7) Application fee for certification in the administration of intra-oral infiltration local anesthesia and intra-oral block anesthesia:
(A) Intra-oral infiltration local anesthesia	$ 10
(B) Intra-oral block anesthesia	$ 25
(8) Biennial renewal fee	$ 40
(9) Restoration fee	$ 50
(10) Compliance resolution fund:
(A) Compliance resolution fund fee	$ 35
(B) Annual compliance resolution fund fee	$ 35
(C) Biennial compliance resolution fund fee	$ 70
