Haw. Code R. § 16-53-23 - Massage therapy
The fees for the board of massage shall be as provided in this section:
| (1) Application fees: | |
| (A) Massage therapist | $ 50 |
| (B) Massage therapy establishment | $ 25 |
| (C) Massage therapy apprentice registration | $ 50 |
| (D) Massage therapy workshop | $ 50 |
| (2) Examination and other examination related fees as determined by agreement between the board and a professional testing service | |
| (3) Original license fees: | |
| (A) Massage therapist | $ 25 |
| (B) Massage therapy establishment | $ 25 |
| (4) License fees: | |
| (A) Massage therapist: | |
| (i) First year of the biennium (original license fee, plus one-half of the biennial renewal fee) | $ 50 |
| (ii) Second year of the biennium (original license fee) | $ 25 |
| (B) Massage therapy establishment: | |
| (i) First year of the biennium (original license fee, plus one-half of the biennial renewal fee) | $ 50 |
| (ii) Second year of the biennium (original license fee) | $ 25 |
| (5) Biennial renewal fees: | |
| (A) Massage therapist | $ 50 |
| (B) Massage therapy establishment | $ 50 |
| (6) Inactive fee | $ 10 |
| (7) Reactivation fee: | |
| (A) Massage therapist | $ 10 |
| (B) Massage therapy establishment | $ 30 |
| (8) Restoration fee for failure to renew license shall be the penalty fee of $100, in addition to all delinquent fees. | |
| (9) Compliance resolution fund: | |
| (A) Compliance resolution fund fee | $ 35 |
| (B) Annual compliance resolution fund fee | $ 35 |
| (C) Biennial compliance resolution fund fee | $ 70 |
