Haw. Code R. § 16-53-29.5 - Occupational therapists

The fees for occupational therapists shall be as provided in this section:

(1) Application/registration fee	$ 50
(2) Biennial renewal fee	$ 50
(3) Inactive fee and reactivation fee	$ 10
(4) Compliance resolution fund fee	$ 35
