Haw. Code R. § 16-53-3.5 - License verification or verification of any other licensing information

Where written verification of State licensure or verification of any other licensing information regarding a licensee is requested, a verification fee shall be assessed by the department and payment shall be payable to "Commerce and Consumer Affairs". The fee for each verification shall be $15; provided that the department may charge a higher verification fee for staff time spent searching for, reviewing, segregating, or extracting information in responding to a more complex verification request when the time spent can be documented to be valued at more than $15.
