Haw. Code R. § 16-53-33.5 - Physical therapy

The fees for the board of physical therapy shall be as provided in this section:

(1) Application fee	$ 50
(2) Examination and other examination related fees as determined by agreement between the board and a professional testing service
(3) Original license fee	$ 30
(4) License fees:
(A) First year of the biennium (original license fee, plus one-half of the biennial renewal fee)	$ 80
(B) Second year of the biennium (original license fee)	$ 30
(5) Temporary license fee	$ 30
(6) Biennial renewal fee	$ 100
(7) Inactive and reactivation fee	$ 10
(8) Restoration for failure to renew license shall be the payment of the delinquent fees plus an amount equal to fifty per cent of the delinquent fee.
(9) Compliance resolution fund:
(A) Compliance resolution fund fee	$ 35
(B) Annual compliance resolution fund fee	$ 35
(C) Biennial compliance resolution fund fee	$ 70
