Haw. Code R. § 16-72-48 - Failure to renew; forfeiture; restoration

Failure to pay the renewal fee when due shall constitute automatic forfeiture of the license. However, a license which has been forfeited for failure to pay the renewal fee may be restored within one year after the date of forfeiture upon compliance with the licensing renewal requirements provided by law and upon written application and payment of the appropriate restoration fees as provided in rules adopted by the director in accordance with chapter 91, HRS. After one year from the date of forfeiture, a license shall not be restored and the person shall be treated as a new applicant and shall meet all the requirements of a new applicant.
