Haw. Code R. § 16-74-61 - Application
Application for a license as manager shall be filed with the commission's executive secretary on a form provided by the commission not less than five days before the regular meeting which could consider such license. Managers, when applying for licenses, must be completely fingerprinted and submit two passport type photographs of themselves at the time of application.
