Haw. Code R. § 16-76-21.02 - Reactivation of inactive license
(a) A licensee on
inactive status may reactivate a license to active status by submitting an
application of reactivation of inactive license on a form approved by the
board.
(b) If inactivation and
reactivation fall within the same biennium, the licensee shall be required to
pay a non-refundable fee of no less than $25 for reactivation of the license.
Fees provided in chapter 16-53, shall supersede the fees in this
section.
(c) However, if activation
is requested at renewal, for the next biennium, the licensee shall be required
to submit payment of the current renewal fee and compliance resolution fund
fee, as provided under section 16-53-27, provided the licensee:
(1) Meets all current renewal
requirements;
(2) Has a current,
active chiropractic license in at least one other state throughout the period
in which the license was inactive in the State; and
(3) All current, active chiropractic licenses
held in other states are unencumbered at the time of application.
