Haw. Code R. § 16-81-6 - Application for examination
(a) An application
for examination shall be filed with the board in accordance with and contain
the information prescribed for in the board's application.
(b) If applicable, the application for
examination shall state that the applicant is presently working under a
temporary permit as an elevator mechanic.
