Haw. Code R. § 16-99-106 - Forfeited registration, certification; restoration
Current through February, 2022
(a) Failure, neglect, or refusal by any
person to pay the respective biennial renewal provider registration fee or
course certification fee shall constitute a forfeiture of the certificate of
registration or certification.
(b)
Any forfeited certificate of registration or certification may be restored upon
satisfaction of all of the following:
(1)
Written application;
(3) Payment of the applicable fees specified
in rules adopted by the director of commerce and consumer affairs pursuant to
chapter
91, HRS.
