Haw. Code R. § 17-1709.2-7 - Requirements for an eligible employer

An eligible employer shall:

(1) Have had no more than fifty employees on January 1, 2 010 employed in the State.

(2) Meet all the requirements of the Hawaii Prepaid Health Care Act.

(3) Have the appropriate licensure to operate in the State.

(4) Provide their taxpayer identification number.

(5) Employ the qualified employee for full-time employment.

(6) Intend to employ a qualified employee full-time for a minimum of twenty-four consecutive months.
