Haw. Code R. § 17-2026-39 - Verification of Information
(a) A provider
agency shall obtain documentation to verify applicant Information relating to
participation in the grant program. This documentation shall include but not be
limited to:
(1) Homeless Status: if at-risk
homeless, an eviction notice from current residence, lease agreement for the
unit from which the participant is being evicted, the unit to which the
participant will move, and the general excise tax license number of landlord of
unit for which the grant will be issued;
(2) Expenses and income, including but not
limited to pay stubs, and verification of' benefits;
(3) For housing-related program assistance, a
copy of the lease agreement for which the assistance will go towards and the
landlord's general excise tax license number;
(4) For sheltered homeless, verification from
the shelter staff at which the applicant is residing; and
(5) For unsheltered homeless, verification
from another provider agency, an outreach worker, case worker, or other
appropriate source.
(b)
An applicant to the grant program who fails to comply with the requirements to
provide verifying documentation shall be ineligible to participate in the grant
program. A provider agency may set time limits as to how long an applicant has
to submit Information.
(c) A
provider agency shall maintain verification documentation of eligibility of a
participant and eligibility of the use of program assistance.
