Haw. Code R. § 4-22-4 - Veterinarian responsibility
(a) Veterinarians
in the State of Hawaii shall report all cases of reportable diseases or disease
agents suspected, observed or diagnosed, to the state veterinarian.
(b) Written or verbal notification of
suspected, observed or diagnosed list A, list B or list I diseases shall be
made according to the following:
(1) List A
diseases and designated list B and list I diseases or disease agents shall be
reported immediately, or within twenty-four hours; and
(2) Other list B and list I diseases or
disease agents shall be reported within the time periods listed in Exhibit A
attached at the end of this chapter.
(c) Notification to the state veterinarian of
list A, list B or list I diseases or disease agents shall include, but not be
limited to, the following information:
(1)
Name of the veterinarian reporting;
(2) Disease or disease agent being
reported;
(3) Species, location and
number of animals involved; and
(4)
Name and address of the owner of the animals.
