Haw. Code R. § 4.02 - Rejection of an Enrollment Application
(a) Any enrollment
application may be partially or fully rejected if it is incomplete or does not
contain all information required to be provided by the
employee-beneficiary.
(b) An enrollment
application shall be rejected if:
(1) The
application seeks to enroll a person who is not eligible to enroll in the benefit
plan for which enrollment is requested;
(2) The application is not filed within the time
limitations prescribed by these rules;
(3) The application contains an intentional
misstatement or misrepresentation of a material fact or contains other information
of a fraudulent nature;
(4) The
employee-beneficiary is ineligible due to outstanding past due contributions or
other amounts to the Fund, or is ineligible due to Cancellation Non-Payment Status;
or
(5) Acceptance of the application
would violate applicable federal or state law or any other provision of these
rules.
(c) Notification shall
be provided to the employee-beneficiary of the rejection of any enrollment
application.
