Haw. Code R. § 6-83-40 - Facility operator registration
Each operator of a subsurface facility, as defined in section 269E-2, HRS, shall register with the one call center by providing the facility operator business name, street address, mailing address, telephone number, e-mail address, a description of its operations, and all other information requested by the commission.
