Ill. Admin. Code tit. 68, § 1330.340 - Application for Licensure on the Basis of Examination
a) An applicant for licensure on the basis of
examination shall submit to the Division a properly completed application on
forms provided by the Division, along with the following:
1) The fee required by Section
1330.20;
2) Certification of graduation from an
approved program of pharmacy (see Section
1330.300); and
3) Proof of successful completion of the
examination approved by the Division (see Section
1330.330).
b) Upon receipt of the items required in
subsection (a), and upon verification by the Division that the candidate meets
all of the requirements for licensure as a Registered Pharmacist, the Division
shall issue a license to practice pharmacy or notify the applicant of the
reason for denial.
