Ill. Admin. Code tit. 20, § 120.90 - Information to be Reported
a)
Employees shall immediately report to their supervisor any information
indicating a violation or attempted violation of criminal laws, or a threat to
the safety and security of the facility, its property or any person, including
information regarding a potential escape.
b) Reports shall be made verbally and, if
requested or if required, reports shall be made in writing in the manner
directed by the employee's supervisor.
