Ill. Admin. Code tit. 20, § 800.60 - Application Fees
a)
The non-refundable application fee shall be $250 per facility and must be
submitted with each application for a license, renewal of a license, or
relocation of a licensed facility .
b) Payment shall be made by check or money
order payable to the Department . A separate check or money order shall be
submitted with each facility application.
c) No application fee shall be required of
any unit of local, State, or federal government.
