Ill. Admin. Code tit. 35, § 604.165 - Monthly Operating Report
a) The community
water supply must prepare an operating report on a form approved by the Agency
as specified in a construction, operating or special exception
permit.
b) An individual set of
operating reports must be maintained for each installation when more than one
source of water with separate chemical addition equipment is used.
c) The operating report must be signed by the
Responsible Operator in Charge and submitted to the Agency within 30 days after
the last day of the month.
d) A
copy of the operating report records must be maintained by the official
custodian of the community water supply.
