Ill. Admin. Code tit. 50, § 930.80 - Life Insurance Buyer's Guide, Language and Content
The form requirements of the Life Insurance Buyer's Guide are found in Exhibit A or Exhibit B of this Part. The reproduction of the Buyer's Guide should be as specified in Exhibit A or Exhibit B, except that the designation "Exhibit A" or "Exhibit B" assigned by the Illinois Department of Insurance should not be used.
