Ill. Admin. Code tit. 68, § 1455.115 - Inactive Status
a) A licensee
desiring to place their license into inactive status shall notify the Division
and provide any completed forms prescribed by the Department.
b) A licensee desiring to be reinstated from
inactive to active status shall notify the Division and provide the following:
1) Forms prescribed by the
Division;
2) The renewal fee as set
forth in Section 1455.320; and
3) Proof of satisfying the continuing
education requirements for the period of time during which the license was
inactive, as provided by Subpart C.
c) The period of time during which a license
may be in inactive status shall not exceed two consecutive renewal
periods.
d) A licensee may not
perform any licensed activities under the Act while their license or credential
is placed on inactive status.
