Ill. Admin. Code tit. 68, § 690.170 - Denial of Initial License or License Renewal
A license application may be denied for any of, but not limited to, the following reasons:
a) failure
to comply with the minimum standards set forth by the Act or Section
690.70;
b) a felony conviction of the owner or
operator of the nurse agency;
c)
failure to provide an accurate statement of financial solvency (see Section
690.40(g));
d) failure to establish personnel policies
and procedures as required by the Act and this Part;
e) failure to complete the application in
full;
f) submission of false or
misleading information to the Department;
g) for any violations of provisions of the
Act or of this Part, if the applicant has previously held a license under this
Act; or
h) for refusal to comply
with enforcement provisions of the Act or this Part, such as refusal to pay a
fine, if the applicant has previously held a license under this Act.
