Ill. Admin. Code tit. 68, § 690.50 - Fee Schedule
a) The
application must be accompanied by a $1,000 fee and a $250 fee for each branch
office or other location in Illinois from which the nurse agency
operates.
b) The fee shall be paid
using the State Treasurer's E-Pay program or any successor program.
