Ill. Admin. Code tit. 77, § 260.2000 - Medical Day Care
a) No more than 12
children shall be served at a time.
b) The facility shall provide services as
necessary to implement and support the child's plan of treatment and overall
needs, including provisions for:
1) Case
management;
2) Fostering maximum
independence of the child; and
3)
Protection of the child's rights, privacy and dignity.
c) The facility shall have one or more
transfer agreements with hospitals to provide emergency care to
children.
d) The facility shall
provide recreational and leisure activities for children during their stay, two
to four hours per day as tolerated by the child.
e) A written summary of the child's stay
shall be sent home with each child. The summary shall contain documentation of
any extreme (positive or negative) occurrences and any changes to the plan of
treatment.
f) All information
related to the child, the child's representative or the child's plan of
treatment is confidential and shall be accessible only to those individuals who
need the information to assure appropriate service delivery.
