Application forms may be obtained by writing to the Illinois Liquor Control
Commission at 100 W. Randolph St., Suite 5-300, Chicago IL 60601 or at 101 W.
Jefferson St., Springfield IL 62702.
b) Application forms shall be signed and
dated.
