Ill. Admin. Code tit. 77, § 385.1020 - Application for License
a) Any
person acting individually or jointly with other persons who proposes to build,
own, establish, or operate a supportive residence for persons with HIV disease
shall submit pre-application information on forms provided by the
Department.
b) Application for a
license to establish or operate a residential facility for persons with HIV
disease shall be made in writing and submitted, with other such information as
the Department may require, on forms provided by the Department.
c) The application shall contain the
following information:
1) The name and address
of the person or business entity who will be the licensee, and if appropriate:
A) The name and address of the corporation,
Board of Directors, Officers and Registered Agent;
B) Documentation certifying the corporation
as a general not-for-profit corporation in accordance with the General
Not-For-Profit Corporation Act of 1986 (Ill. Rev. Stat. 1991, ch. 32, par.
101.01 et seq.) [805 ILCS 105 ];
2) The names and locations of the facilities
for which a license is sought;
3)
The name of the person or persons under whose management or supervision the
facility will be operated;
4)
Ownership disclosure as required in Section
385.1070.
