Ill. Admin. Code tit. 92, § 1030.27 - Identification Cards for Youth in Care
a) An applicant for an Identification Card
age 16-20 who is a youth for whom the Department of Children and Family
Services (DCFS) is legally responsible for or is a foster child and who submits
a properly completed DCFS form number CFS 906/906-1 shall be issued an
Identification card at no cost.
b)
The CFS 906/906-1 form must be signed by a representative of DCFS and the
applicant's caseworker and/or provider.
c) To be accepted, the CFS 906/906-1 form
must contain the following information:
1)
Complete name and residential address of applicant;
2) DCFS Client ID number;
3) Provider name and address; and
4) Provider signature and current
date.
