Ill. Admin. Code tit. 92, § 15.50 - Application Procedure
Applications for loans under this Part shall be made in writing on forms that are approved by the Department. Evidence must be provided with the application that the governing body of the Owner has approved the loan request. An example of such evidence would be a certified resolution by the governing body of the Owner.
