An establishment may contest adverse action taken pursuant to
this chapter by submitting a request for hearing to the department within 30
days of the mailing or service of the department's action. Appeals and hearings
are governed by 481-Chapter 9. For contractors, license holders shall have the
opportunity for a hearing before the local board of health. If the hearing is
conducted before the local board of health, the license holder may appeal to
the department and shall follow the process for review in rule
481-9.3
