Unless accepted by an order of the department, every person
acting as a principal or agent for another or independently engaged in the
production, storage, transportation (except railroad), refining, reclaiming,
treating, marketing, or processing of oil or gas, or engaged in the exploration
for or production of metallic minerals, shall file with the department on a
form prescribed by the department: the name under which the business is being
operated or conducted; the name and post office address of the person, the
business or businesses in which engaged; the plan of organization, and in case
of a corporation, the law under which it is chartered; and the names and post
office addresses of any persons acting as trustees together with the names of
the manager, agent or executive thereof, and the names and post office
addresses of officers thereof. In the event that business is conducted under an
assumed name, the report shall show the names and post office addresses of all
owners in addition to the other information required.
Immediately after any change occurs as to facts stated in the
report filed, a supplementary report shall be filed with the department with
respect to the change.
