Iowa Admin. Code r. 641-99.8 - Abstraction and certification by state registrar
The state registrar or the registrar's designated representative shall abstract on the delayed certificate of birth a description of each document submitted to support the facts on the delayed birth certificate. This description shall include:
1. The title or description of the
document;
2. The name and address
of the affiant if the document is an affidavit of personal knowledge or of the
custodian if the document is an original or certified copy of a record or
certification statement;
3. The
date of the original filing of the document being abstracted;
4. The information regarding the birth and
parentage contained in the document.
