Iowa Admin. Code r. 645-61.4 - Establishment license renewal
(1) The biennial
license renewal period for an establishment license shall begin on January 1 of
every odd-numbered year and end on December 31 two years later.
(2) A renewal of license notice shall be
electronically mailed to the owner of the establishment prior to the expiration
of the license. Failure to receive the renewal notice shall not relieve the
owner of the obligation to pay the biennial renewal fee on or before the
renewal date.
(3) An establishment
that is issued a license within six months of the license renewal date will not
be required to renew the license until the next renewal two years
later.
(4) The establishment owner
shall submit the completed application with the renewal fee to the board office
before the license expiration date.
(5) An establishment shall be in full
compliance with this chapter and 645-Chapter 63 to be eligible for renewal.
When all requirements for license renewal are met, the establishment shall be
issued a license renewal.
(6) If
the renewal fee and renewal application are received in the office after the
license expiration date, but within 30 days following the expiration date, the
late fee for failure to renew before expiration shall be charged.
