Iowa Code r. 101-1.7 - Forms
The following forms are available at the council office without charge:
(1)
Monthly remittance
form. This form is used to accompany the receipt of checkoff moneys to
the council.
(2)
Refund
application form. This form must accompany any request for refund of
checkoff moneys remitted.
