Iowa Code r. 481-571.4 - Late renewal fees
A licensee, registrant or permit holder who fails to renew a license, registration or permit by the expiration date is subject to late renewal fees as described in this rule.
(1)
Dental or dental hygiene license
or faculty permit.
a. Renewals
completed between September 1 and September 30 are subject to a late fee of
$100, in addition to the renewal fee.
b. Renewals completed between October 1 and
October 31 are subject to a late fee of $150, in addition to the renewal
fee.
(2)
Dental
assistant registration or dental radiography qualification.
a. Renewals completed between September 1 and
September 30 are subject to a late fee of $20, in addition to the renewal
fee.

October 1 and October 31 are subject to a late fee of $40, in addition to the
renewal fee.
