Kan. Admin. Regs. § 100-6-4 - Applications for licensure by examination
Applicants for licensure by examination shall submit the following requirements not later than sixty (60) days preceding the date of examination:
1. A written
application, on a form prescribed by the board with the full name and address
of the applicant subscribed thereto.
2. A photograph of the applicant, exactly 3 x
4 inches in size, taken within ninety (90) days prior to the making of
application and a certificate of the photographer upon the reverse side showing
the date and place such picture was taken. A thumb print on back of photograph
shall be taken by any law enforcement agency or in the office of the board of
healing arts and certified by the person taking the print.
3. An affidavit specifying in detail that the
applicant has met the following minimum educational requirements:
(a) A graduate of an accredited healing arts
school or college stating the name and location of such school or college and
the date of graduation. This affidavit shall further state that the applicant
is the identical person attending the school and receiving the degrees claimed
in such affidavit.
(b) A certified
copy of accredited healing arts school or college diploma.
4. Evidence of proficiency in
basic science issued by the national board of medical examiners, the national
board of examiners of osteopathic physicians and surgeons or the national board
of chiropractic examiners or such other examining body as may be approved by
the board or in lieu thereof pass such examination as the board may require in
the basic science subjects.
5. A
certificate of the applicant's good moral character signed by two (2) reputable
teachers or practitioners of the healing arts licensed in some state of the
United States personally acquainted with the applicant.
