Kan. Admin. Regs. § 26-52-18 - Case management services
(a) Each licensee
shall develop and implement policies and procedures to provide case management
services to each patient admitted to the center. A case manager shall
facilitate, assist, and coordinate processes and services for the patient,
including the following:
(1) Assisting the
behavioral health professional in development and implementation of the
treatment plan for patients admitted to the center for more than 24 hours by
establishing goals for the patient while admitted to the center and upon
discharge from the center;
(2)
identifying services needed by the patient after discharge to support the
patient's efforts to meet goals established in the treatment plan;
(3) arranging for transportation of a patient
if the clinical director or designee determines the patient's medical needs
exceed the level of medical care that can be safely provided at the
center;
(4) arranging for
reasonable accommodation of the patient's transportation needs upon discharge
of the patient pursuant to
K.S.A. 59-29c08, and amendments thereto;
and
(5) arranging for referral of
the patient to the appropriate community mental health center and other
services for follow-up care upon discharge of the patient.
