Kan. Admin. Regs. § 28-35-181s - Specific licenses for well logging
Each application for a specific license for the use of licensed material in well logging shall be considered for approval only if the application contains the following:
(a) A description of the training program for
logging supervisors and logging assistants that includes the following:
(1) The content of and method for initial
training;
(2) on-the-job training;
(3) annual safety reviews provided
by the licensee;
(4) the means
that the applicant will use to demonstrate the logging supervisor's knowledge
and understanding of and ability to comply with these regulations, the license
conditions, and the applicant's operating and emergency procedures; and
(5) the means that the applicant
will use to demonstrate the logging assistant's knowledge and understanding of
and ability to comply with the applicant's operating and emergency procedures;
(b) a copy of the
written operating and emergency procedures required by 28-35-383 or an
outline or a summary of the procedures that includes the radiation safety
aspects;
(c) a description of the
program, which shall include records, for annual inspections of the job
performance of each logging supervisor to ensure that these regulations, the
license conditions, and the applicant's operating and emergency procedures are
followed. The inspection records shall be retained for three years after each
annual internal inspection;
(d) a
description of the applicant's overall organizational structure as it applies
to the radiation safety responsibilities in well logging, including any
delegation of authority and responsibility; and
(e) the manufacturer's name and model numbers
of the leak test kits to be used, if an applicant desires to perform leak
testing of the sealed sources. If the applicant desires to analyze the
applicant's own wipe samples, the application shall include a copy of the
procedures to be followed. The procedures shall include the following:
(1) The instruments to be used;
(2) the methods of performing the analysis;
and
(3) the applicable experience
of the person who will analyze the wipe samples.
