Kan. Admin. Regs. § 69-12-9 - Report of injuries
(a) The tanning
facility operator shall forward to the board of cosmetology, a written report
of any injury, within five working days of its occurrence or knowledge thereof.
The report shall include:
(1) the name of the
affected individual;
(2) the name
and location of the tanning facility involved;
(3) the nature of the injury; and
(4) any other information considered relevant
to the situation.
