Kan. Admin. Regs. § 80-5-10 - Designation of beneficiaries
A member may designate any natural person or persons, trust, or estate as beneficiary. This designation may be made on the designation of beneficiary form or a written document filed with the system. Any documents other than the forms furnished by the system shall state the name, address, relationship, and date of birth or other information of sufficiency to establish the identification of the individual so designated. The written notice shall be dated and signed by the member, and the signature shall be witnessed by an individual other than a designated beneficiary. Attorneys-in-fact, conservators, and guardians shall not change any designation of beneficiary.
