La. Admin. Code tit. 52, § I-1313 - Disclosure Forms Filed Pursuant to R.S. 27:96(A) and R.S. 27:261(A)
A. Disclosure forms
filed pursuant to
R.S.
27:96(A) and
R.S.
27:261(A) shall:
1. be in writing and on a form provided by
the board or a form which is substantially similar to the form provided by the
board;
2. be filed no later than
five days prior to the public officer's performance;
3. contain:
a. the name and address of the public
officer;
b. the position held by
the public officer;
c. the date,
time and place of the performance;
d. the amount of compensation the public
officer has contracted to receive for the performance; and
e. the identity of the person or entity
providing the compensation;
4. include a copy of the contract.
B. The executive secretary shall
maintain these forms suitably indexed.
