02-373 C.M.R. ch. 1, § 7 - PROCESS FOR CONVERSION OF AN INACTIVE STATUS LICENSE OR ADMINISTRATIVE LICENSE TO AN ACTIVE STATUS LICENSE
1. The Board, or if
delegated, Board staff may convert an inactive status license to an active
status license for an applicant who:
A.
Submits an administratively complete application requesting an active status
license on forms approved by the Board;
B. Pays the appropriate license conversion
fee;
C. Provides evidence of having
met the Board's requirements for CME;
D. Demonstrates continuing clinical
competency as required by this rule;
E. Meets the examination requirement; and F.
Has no cause existing that may be considered grounds for disciplinary action or
denial of licensure as provided by law.
2. In the event that there is any question
regarding CME credits or active clinical practice, the application will be
presented to the Licensure Committee which will make a recommendation regarding
the application to the full Board.
