29-250 C.M.R. ch. 162, § 5 - LEGAL NAME AND ADDRESS
1. Identification
Numbers will be used to identify and track all applicants and registrants. Such
numbers may include an applicant's or registrant's Social Security Number
(SSN), Federal Employer Identification Number (FEIN), USDOT Number (see Section
6) , or Motor Carrier Number (MC
Number). The Social Security Number may be used for an individual applicant or
registrant.
2. Applicants must
provide their full legal name as filed with the United States Department of
Transportation's Federal Motor Carrier Safety Administration (FMCSA). If the
applicant is an individual, the applicant must provide their full name and date
of birth. The legal name of the applicant as provided on the Schedule A will
appear on the registration cab card.
3. The legal name as listed on the Uniform
Application must be consistent with the legal name provided on other
applications for motor carrier credentials as well as with any applicable
corporate filing. The applicant may be required to rectify any legal name
discrepancies before an apportioned registration will be issued.
4. Applicants must provide a phone number and
a full address including a street and street number if applicable. If the
address is a rural route, the applicant must include a box number.
5. An application without sufficient name,
address, or contact information may be returned to the applicant.
6. Registrants must report any change of
address to the IRP Unit within 30 days (see Section 6).
