Md. Code Regs. 10.03.01.15 - Certificate of Birth Resulting in Stillbirth

A. The Secretary shall prepare a Maryland certificate of birth resulting in stillbirth upon the request of the parent or parents of a stillborn child.
B. In order for the Secretary to prepare a Maryland certificate of birth resulting in stillbirth:
(1) A fetal death shall be registered pursuant to Regulation .05A of this chapter; and
(2) The parent or parents requesting the certificate of birth resulting in stillbirth shall be named on the registered fetal death certificate.
C. Information on Certificate.
(1) Except as provided in §C(2) of this regulation, the Secretary shall include on the certificate of birth resulting in stillbirth the following information as it appears on the fetal death certificate:
(a) Name of fetus;
(b) Date of delivery;
(c) Mother's maiden name;
(d) Mother's married name;
(e) Father's name; and
(f) Jurisdiction of delivery, specifying:
(i) A county; or
(ii) Baltimore City.
(2) The Secretary shall include on a certificate of birth resulting in stillbirth the words, "Not a Certificate of Live Birth".
(3) If the parent or parents do not wish to provide a name for the stillborn child, the Secretary may not include on the certificate of birth resulting in stillbirth any name for the stillborn child other than that appearing on the fetal death certificate.
(4) If the parent or parents want to add a name to appear on the certificate of birth resulting in stillbirth for the stillborn child that does not appear on the fetal death certificate or amend the name for the stillborn child, the parent or parents shall follow the procedure for additions and amendments to a name on a birth certificate, as set forth in Regulation .03B of this chapter.
