Md. Code Regs. 13A.12.01.09 - [Effective until 4/1/2024] Certificate Form

A. Basic Information. All certificates shall have the same basic information which shall include all of the following information:
(1) Applicant's name and identification number;
(2) Period of validity;
(3) Highest degree earned;
(4) Type of certificate;
(5) Specific certification areas; and
(6) Notice of responsibility to renew.
B. Period of Validity. A certificate shall remain valid to the expiration date designated on the current certificate form under "Period of Validity".
C. Highest Degree Earned. The highest degree earned shall be listed on a certificate if the applicant provides official transcripts to verify completion of the degree from an IHE. Any change in the highest degree shall be added only at the renewal date of a certificate.
D. Specific Certification Areas. A certificate shall name one or more of the following:
(1) Grade levels such as early childhood, elementary, middle, secondary, and PreK-12;
(2) Content area or subject area such as English, music, or physical education; or
(3) Job specific designation such as administrator, supervisor, or specialist in which the applicant is certified.
