310 CMR, § 40.0333 - How to Notify
(1)
Two
Hour and Seventy-Two Hour Notifications. Persons described in
310
CMR 40.0331(l) shall:
(a) notify the Department of a release or
threat of release specified in
310
CMR 40.0311 through
40.0314, by calling a
telephone number published by the Department and designated for that purpose
and orally providing to the Department the information specified in
310
CMR 40.0334; and
(b) within 60 days thereafter, submit a
completed Release Notification Form, as described in
310
CMR 40.0371, to the Department office located
in the DEP region in which the release or threat of release occurred. Where
appropriate, the Release Notification Form may be accompanied by a Permanent or
Temporary Solution Statement, as described in
310
CMR 40.1000.

120 Day
Notifications. Persons described in
310
CMR 40.0331(1) shall notify
the Department of a release specified in
310
CMR 40.0315 by submitting a completed Release
Notification Form, as described in
310
CMR 40.0371, to the Department office located
in the DEP region in which the release occurred. Where appropriate, the Release
Notification Form may be accompanied by a Permanent or Temporary Solution
Statement, as described in
310
CMR 40.1000.
