Mich. Admin. Code R. 325.10204 - Initiation of contested case hearing
Rule 204.
(1) Contested case hearings may be initiated by the chief of the bureau of environmental and occupational health. Except in the case of suspension or revocation of a license, permit, order, variance, or exemption, the chief of the bureau shall initiate a contested case hearing by notice mailed by certified mail not less than 21 days prior to the hearing.
(2) A person requesting a contested case hearing shall file a petition with the division in Lansing, Michigan. The petition shall state the legal authority under which the hearing is requested, a brief statement of the matters asserted, a statement of the relationship of the petitioner to the issue, and a statement of relief sought.
