Mich. Admin. Code R. 325.22146 - Nontransport prehospital life support operation; relicensure
Rule 146.
(1) A
nontransport prehospital life support operation shall complete an application
for relicensure and return the completed application to the department before
the date of license expiration. Failure to receive a notice for relicensure
from the department does not relieve the licensee of the responsibility to
apply for relicensure.
(2) The
license of a nontransport prehospital life support operation and its vehicles
shall expire on the same date.
(3)
An application for licensure renewal received by the department after the
license expiration date, but within 60 calendar days after the expiration date
of the license, shall require the nontransport prehospital life support
operation to comply with section 20936 of the code.
(4) A nontransport prehospital life support
operation may provide emergency medical services during the 60 days following
its license expiration date, whether or not the department has received an
application.
(5) An application for
licensure renewal not received by the department within 60 calendar days
following the date of license expiration shall be considered revoked, effective
on the sixty-first day.
(6)
Reinstatement of the nontransport prehospital life support operation and
vehicle licenses shall require completion of a new application for licensure,
including all fees prescribed in section 20936(1) and (2) of the
code.
