Rule 4.
(1) All
vital records forms prepared and distributed by the state registrar for
reporting vital events and vital statistics shall be surrendered to the state
registrar upon demand.
(2) Only
forms furnished or approved by the state registrar shall be used to report
vital events and vital statistics or to make copies thereof.
(3) The forms prescribed and distributed by
the state registrar shall be used only for official purposes.
